Comments:
The licensing inspector conducted an unannounced monitoring inspection on Tuesday, March 28, 2017 from approximately 10:15 am to 12:27 pm to determine compliance with licensing standards. During this inspection, there were 6 children totaling 16 points being cared for by two caregivers. The children were observed in the designated rooms in the family day home playing with age appropriate toys, and participating in various glow in the dark activities. The licensing inspector completed observation of the staff completing diapering of the children, preparing for lunch transition to include the children completing bathroom breaks and handwashing. The diapering area and bathroom were deemed in compliance with licensing standards. The children and staff were observed by the licensing inspector engaged in interactive dialogue while sitting at the table to be served for lunch. The licensing inspector observed the children participating in saying grace prior to eating their lunch. The children's lunch consisted of taquitos, spanish rice, baked tomatoes, corn on the cob, and pineapple. The caregivers were observed by the licensing inspector providing age appropriate supervision and addressing the children's overall needs. Interviews were held with the caregivers throughout the inspection. Additionally, when appropriate the inspector interacted with children in care. The designated areas for childcare were deemed appropriate and equipped with age appropriate equipment and materials to meet the needs of the children. The inspector observed the required postings to include the license, recent inspection summary, emergency contacts, and menu. The first aid kit and emergency supplies, posted telephone numbers, fire and shelter-in-place drills, emergency preparedness plan were inspected and determined to be in compliance with the licensing standards. During this inspection, four children's, the provider's, two substitute providers'/assistants' records were reviewed for compliance of standards. There are currently no medications being administered by the provider. The licensing inspector completed some but not all licensing standards during this inspection. Standard #: 22VAC40-111-100-A
Description: Based on review of the children's records, the provider did not obtain documentation of a physical examination prior to the child's attendance or within 30 days after the first day of attendance.

Evidence:
1. Child #1's record, start date 02/01/16 was missing documentation of a physical examination.
2. Child#4's record, start date 02/06/17 was missing documentation of a physical examination. The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

Plan of Correction: The provider will request and obtain a copy of child's physical from the parent.
