54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was initiated today at 12:30pm. The physical plant, 2 caregiver/household member files, 4 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today. There were 8 children, totaling 20 points, under the supervision of 2 caregivers. During the inspection, children were observed eating lunch, watching an educational video, cleaning up for and resting at nap time. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served pasta with beef and tomato sauce, peaches and milk. The Provider is currently certified in MAT (medication administration training), CPR and First Aid. There are no children with any medication needs at this time. Standard #: 22VAC40-111-60-B Description: Based on file review, 2 out of 4 children's records do not have all required information. Evidence 1. Children #3 and #4 do not have two emergency contacts in case the parents cannot be reached. They both have 1 emergency contact listed on their file. Plan of Correction: The Provider will ask the parents for 1 more contact for each child.
Standard #: 22VAC40-191-40-D-1-b Description: Based on file review, caregiver #2 does not have the findings of a criminal history record check (CRC) before 30 days of employment at the facility ends. Evidence 1. Caregiver #2 began working at the FDH in March 2015. There is no completed CRC on file. Plan of Correction: A new request was sent on 2/27/17. This will be followed up on if not received by tomorrow.
