Timely recording attendance as children arrive and depart was discussed with caregiver 2. Confidential information on the Supplemental Information sheet was discussed with the provider. The Supplemental sheet is for provider information only and is not to be posted.
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 1:30 PM and departed at 3:30 PM. Nap time, afternoon snack and activities were observed. Thirteen children's records were reviewed. It was reported that no medication has been administered to children in care. No children's injuries were reported. The children are currently not using the back yard. The children are walked to the local park for outdoor play. An exit meeting was held with the provider, during which the plans of correction were obtained for the violations cited. Please contact the inspector if there are any questions.
Standard #: 22VAC40-111-60-B Description: Based upon review of thirteen children's records, the provider has not ensured that each child's record includes all required information. Evidence: 1. The record provided for child 10 did not have indicated the child's first date of attendance. 2. The enrollment information page in the record provided for child 1 has not been signed/dated by the parent/guardian. Plan of Correction: The provider responded with the following: 1. The first date of attendance was added to the record of child 10. 2. The parent of child 1 will be required to sign and date the child's enrolment information page.
Standard #: 22VAC40-111-280 Description: Based upon observation, the provider has not ensured that potentially poisonous substances are stored in areas that are inaccessible to the children. Evidence: The child proof latch on the storage cabinet under the sink in the kitchen was not secured. Numerous cleaning agents, Easy Off Oven cleaner and Raid insecticide spray are stored in this cabinet. Child 8 was observed crawling into the kitchen area. Plan of Correction: The provider responded with the following: All caregivers will be reminded that potentially hazardous substances must be kept in areas with child proof latches, making them inaccessible at all times.
