Comments:
An unannounced monitoring/renewal inspection was conducted today from 10:45am-12:45pm. There were 7 children (15 points) directly supervised by the provider and two assistants. The home, 3 staff records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, playing outdoors with bubbles and preparing for lunch. There was an abundant supply of books, toys, and materials for the children. Violations:
Standard #: 22VAC40-111-240-A
Description: Based on observation of the home, areas and furnishings of the family day home, inside and outside, were not maintained in a clean, safe, and operable condition. Evidence: 1. Linoleum flooring in the basement bathroom was beginning to crack behind the toilet. 2. The toilet in the basement bathroom had not been cleaned at the time of inspection. Urine was on the rim of the toilet. 3. A purple car in the outside play area had a broken steering wheel and the plastic on the tires was beginning to crack.

Plan of Correction: I will have my handyman fix the tile in the bathroom and clean the bathroom today. The purple car will be discarded.

Standard #: 22VAC40-111-500-C
Description: Based on observation, the stationary outdoor playground equipment was placed over a hard surface. Evidence: The mulch beneath the swings on the outdoor stationary playground equipment has worn away leaving a hard dirt surface.

Plan of Correction: I will fill in with mulch or replace with rubber mats.
