Comments:
An unannounced monitoring visit was conducted today beginning at 9:10am. The provider and 4 children (13 points) were present. The physical space, emergency evacuation drills, 4 children's records, provider records and household member records were reviewed. All required documents were posted. No medications were on site. The children were observed painting, engaged in free play and interacting with the provider. All interactions were positive. The space was clean, organized and contained an abundance of age appropriate materials. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-111-120-A
Description: Based on record review, the provider did not maintain required records for self and one household member. Evidence: The Criminal History Record and Central Registry search for both expired in February 2017. The Sworn Disclosure Statement for both expired in April 2014.

Plan of Correction: I sent the background forms in. I will call today to follow up. I will fill out the Sworn Disclosure Statements today.
