Technical Assistance:
Reviewed with applicant children records requirements. Inspector will contact the provider about the front yard usage for outdoor play.

An announced initial inspection was conducted on April 12, 2017, from approximately 10:30 am-12:45pm . Three rooms and bathroom will be utilized for the children in care. The applicant will provide care for children ages 6 weeks through 12 years. The following items were reviewed as a part of the inspection: health and safety, physical plant, required documentation, and emergency supplies, procedures and required postings. The applicant will not be administering medication or providing transportation. The inspection will be reviewed with the Licensing Administrator regarding issuance of a conditional license. Exit interview was conducted with the applicant.

Standard #: 22VAC40-111-770
Description: Based on observation and interview, the applicant did not ensure a working portable battery-operated weather band radio, and a working battery operated flashlight. Evidence: During the initial inspection conducted on April 12, 2017 , the applicant had a solar /wind-up radio. The radio is not battery operated. The applicant did not have a flashlight in the home.

Plan of Correction: Applicant will purchase a battery operated radio. Applicant did not have access to the shed where the flashlight was located. The flashlight will have accessible at all times.

Description: Based on observation, the applicant did not ensure there were posted telephone numbers for: 1- 911 or local dial number for police, fire, and emergency medical responders 2- Responsible person for emergency backup care numbers 3- Regional poison control center Evidence: During the initial inspection conducted on April 12, 2017, the emergency numbers were not posted.

Plan of Correction: The applicant corrected during inspection .
