22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced focused monitoring inspection was conducted on March 30, 2017, between the hours of approximately 11:00 a.m. and 1:30 p.m., to follow up on violations identified on the Intensive Plan of Correction (IPOC). Eighteen (18) children were enrolled and nine children were in the care of the provider and one assistant during the on site inspection. Twelve (12) children's records, two caregivers' records and one household member's record were reviewed and/or updated. No repeat violations were cited. Refer to the violation notice for exceptions to compliance identified during this inspection. The subsidy requirements were discussed; please revise your emergency plan to comply with the new requirements. Standard #: 22VAC40-111-120-B Description: Based on review and/or update of two caregivers' records and discussion with caregiver #1 on March 30, 2017, one caregiver's record did not include verification of age. Evidence: 1. Review of caregiver #2's record, on this date, determined that verification of age was not on file. 2. Caregiver #1 confirmed the lack of age verification on file for caregiver #2. Plan of Correction: Verification of age will be obtained today and included in the record.
Standard #: 22VAC40-111-170-A Description: Based on review/update of two caregivers' records and one household member's record, the facility failed to obtain a current tuberculosis (TB) screening form at the time of hire or prior to contact with children for one caregiver and one household member. Evidence: 1. Review of caregiver #2's record, on this date, determined that the TB screening was dated 3/27 2017 and the date of hire was 2/22/2017. 2. Review/update of one household member's record, on this date, determined that a TB screening was not on file. 3. Caregiver #1 confirmed the date of hire and and the TB screening date for caregiver #2 and the lack of a TB screening for the household member. Plan of Correction: In the future, TB exams/screenings will be obtained at the time of hire/prior to coming into contact with children.
Standard #: 22VAC40-191-50-A-3-a Description: Based on review and/or update of two caregivers' records and one household member's record and discussion with caregiver #1 on March 30, 2017, the facility failed to ensure one household member's criminal history record clearance had a satisfactory finding. Evidence: 1. Review of one household member's criminal history record clearance, on this date, revealed that the clearance included a conviction of a barrier crime. 2. Caregiver #1 confirmed the results indicated on the criminal history record clearance. Plan of Correction: The household member will no longer remain a resident of the home and will relocate.
