The licensing inspector conducted an unannounced renewal inspection on Monday, April 17, 2017 from 4:55pm to 6:15pm. The children were observed eating dinner upon the inspector?s arrival, and they were later observed playing board games and being picked up by their families. Today, there were seven children, totaling five points, present with two caregivers. Dinner consisted of: baked chicken, green beans, macaroni and cheese, apple sauce, and milk. The provider does not administer medication. The provider does not provide transportation. The provider does not have pets. Three children?s records, two caregivers? records and one household record were reviewed during the inspection. The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-111-760-A-1 Description: Based on observation on 04/17/2017, the provider did not ensure that the first aid kit contained the required items. EVIDENCE: The first aid kit did not have a thermometer. Plan of Correction: Will get it tomorrow,04/18/2017.
Standard #: 22VAC40-111-830-C Description: Based on observation and caregiver interview on 04/17/2017, the provider did not ensure that the emergency drills were documented. EVIDENCE: 1. The most recent emergency drill documented was on 01/31/2017. 2. Caregiver #1 stated that the drills were completed, but not written down. Plan of Correction: Corrected on site. Documented the dates.
Standard #: 22VAC40-111-100-A Description: Based on a review of children's records on 04/17/2017, the provider did not ensure that a physical examination was obtained prior to a child's attendance or within 30 days after the first day of attendance. EVIDENCE: 1. The record of Child #1, who started at the family day home on 09/02/2014, did not have documentation of a physical examination. 2. The record of Child #2, who started at the family day home on 02/06/2017, did not have documentation of a physical examination. Plan of Correction: Will notify families tonight and request to have physicals by Friday, 04/21/2017.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records on 04/17/2017, the provider did not ensure that one of three records reviewed contained the required information. EVIDENCE: 1. The record of Child #1 did not have a telephone number listed for the place of employment documented for the child's mother. 2. The record of Child #1 did not have an address documented for the first emergency contact listed. Plan of Correction: Will contact parent today to give info by Friday, 04/21/2017.
Standard #: 22VAC40-111-280 Description: Based on observation on 04/17/2017, the provider did not ensure that potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates shall be stored in areas inaccessible to children. EVIDENCE: Cleaning supplies such as disinfectant spay were observed on a shelf over the dryer, and a plastic container on top of the dryer. There were school age children in care. Plan of Correction: Will rearrange a cabinet and put supplies in there with a safety lock.
