Reminded that the bird must not be accessible to the children. Background checks must be renewed every 3 years. TB test renewed every 2 years.
The following inspection took place between the hours of 9:00 am through 12:00. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this announced initial inspection there were no children in attendance. No children records were reviewed, the providers record was reviewed. The standards and state model forms were reviewed. The home and childcare area was inspected. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-160 Description: Based on a review of household member records, one household member does not have a completed background check. Evidence: The provider does not have one household members criminal record report. Plan of Correction: The Alexandria Family Service supervisor was contacted during this inspection, the provider will pick up the original report from the Alexandria office.
