Comments:
An unannounced renewal inspection was conducted today. Observed 7 children (14-points) directly supervised by the provider. Ratio and supervision were in compliance. A sample of 2 children's records, and provider and household member records were reviewed. The physical plant, and emergency procedures were also reviewed. No medications were reported on site. The children were engaged in various activities, to include: parent drop off, free play, reading, singing happy birthday to a friend and decorating baskets for an egg hunt. The interior of the family day home was found to be clean, organized, and well supplied with toys and equipment for the children. Areas of non-compliance are identified in this report. Standard #: 22VAC40-111-830-C
Description: Based upon record review, required documentation of monthly evacuation drills were incomplete. Evidence: There were no drills documented for the months of; January 2017,February 2017 and March 2017.

Plan of Correction: Provider will make sure to document all drills as required and complete 2 shelter in place drills and document each year.

Standard #: 22VAC40-111-180-A
Description: Based upon record review, The provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department. Evidence: Provider and household member record both contained expired TB results (6/2015).

Plan of Correction: Provider could not locate current TB test at the time of the inspection. Will find or obtain new test.
