Comments:
Today an unannounced monitoring visit was conducted with the Provider. This visit took place in the morning between 9:20-11:30 there were 2 children present with the Provider for a total of 5 points within the supervision guidelines. The children were observed having morning free play, nap, blocks, cars, and lunch. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, attendance, menu and emergency procedures. Violations:
Standard #: 22VAC40-111-50-A
Description: Based on observation of the children present, the provider did not have written record of children. Evidence: The provider did not have written attendance record for the children in care.

Plan of Correction: Provider stated that the children had just arrived and she did not fill it out for today. Provider completed during this inspection.

Standard #: 22VAC40-111-320-B
Description: Based on observation of the physical plant, a outlet was missing a protective cover. Evidence: In the kitchen, a outlet along the wall did not include a protective cover.

Plan of Correction: Provider covered the outlet during this inspection.

Standard #: 22VAC40-80-120-E-2
Description: Based on walk thu of physical plant, the most recent licensing inspection was not posted. Evidence: Provider did not have the last licensing inspection from December 2016 posted.

Plan of Correction: Provider stated that she will make sure it is posted.
