Technical Assistance:
The provider inquired about adjusting her hours of operation to include nights and weekend care.

An unannounced mandated renewal inspection was conducted on 4/19/2017 in the Family Day Home. There were 12 children in care with the provider and an assistant present. Based on the ages of the children, the points totaled 31. During this inspection the LI's observed varied activities: rest time, children at play, cleaning and sanitizing of the tables and high chairs, hand washing, afternoon snack, and the arrival of school-aged children. A sample of records were reviewed: 3 caregivers' records and 3 children's records. A prescribed medication and authorization to administer was observed; both care givers are Medication Administration Trained (MAT) certified. The childcare areas were clean and age-appropriately equipped. Finding of today's inspection were reviewed at exit. The inspection was conducted from approximately 12:10 p.m. to 4:10 p.m. Thank you for your time today. If you have any questions please contact the Inspector at 540-309-2310.

Description: Based on reviewed records, documentation of the Tuberculosis test for a caregiver was not signed by a physician, physicians' designee, or an official of the local health department. Evidence: The document reviewed in the record of caregiver 3 for the TB test indicated the test was administered at a health facility, but was not read by the same facility. The TB test was read by an Registered Nurse who is employed at a local hospital, it could not be verified that the individual was acting as the Physicians' designee.

Plan of Correction: The Provider will request documentation of a verified TB screening for the caregiver for record retention.
