An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 8 children, totaling 19 points. The children were observed singing songs and dancing, practicing their handwriting skills, and engaging in creative play. The children were to be served chicken soup with broccoli and peas, wheat bread, and milk for lunch later today. This inspection was completed between approximately 9:30 am and 11:40 am. Standard #: 22VAC40-111-800-B Description: Based on a review of the emergency preparedness plan, it was determined the provider did not conduct a review of this plan as required. Evidence: The provider did not conduct an annual review of the emergency preparedness plan. Plan of Correction: I will do the review with my assistant this weekend.
Standard #: 22VAC40-111-830-B Description: Based on a review of emergency drills, it was determined the provider did not conduct at least 2 shelter-in-place drills during the past year. Evidence: There is no documentation showing that the provider conducted shelter-in-place drills during the past year. Plan of Correction: I will document the two drills this year.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, it was determined that five records were not complete. Evidence: 1. Child record #1-#5 do not include documentation showing that the provider viewed each child's original birth certificate. 2. Child record #1 does not include: a signed "Liability Insurance" document; a signed "Medication Policy" document. 3. Child record #4 does not include: the mother's work phone number; the parent's signature on the enrollment registration form. 4. Child record #5 does not include documentation of the parent's annual review of the child's contact information. Plan of Correction: All parents will be asked for the missing information.
Standard #: 22VAC40-111-180-A Description: Based on a review of caregiver health records, it was determined that one repeat TB Health Screening has expired. Evidence: The provider's TB Health Screening expired on 4/17/17. Plan of Correction: I will get a repeat Health Screening completed.
Standard #: 22VAC40-111-240-A Description: Based on observations, it was determined that one area of the home has not been maintained as required. Evidence: On the floor next to the children's toilet, the provider has stored a wet towel that they use to wipe the bathroom floor. Plan of Correction: The towel will be moved and stored somewhere else out of the reach of the children.
Standard #: 22VAC40-191-40-D-1-c Description: Based on a review of caregiver background checks, it was determined that two repeat background checks have expired. Evidence: 1. The provider's Sworn Disclosure Statement expired on 6/12/16. 2. The Criminal Record Check for one assistant caregiver expired on 8/29/14. Plan of Correction: Both background check forms will be completed today.
