Technical Assistance:
1. Background checks and a TB screening are needed when household children turn 18 years old. 2. Reviewed forms that were lacking information from the Provider, and discussed the possible problems with having parents sign blank documents.

Comments:
Conducted an unannounced monitoring inspection 12:50-1:35pm. There were no children present during this time. The daycare areas of the home were found to be clean, safeguarded appropriately, and sufficiently supplied with toys and equipment for the children. Each child brings food from home daily. No medications reported to be on-site, and the Provider stated that she has a "No Medication" policy. Record keeping violations found. Description: Based on records reviewed, it was determined that 2 of 2 children's records did not include the following required information. Evidence: 1. Child #1 - No name, address and telephone number for 1 of 2 emergency contacts, and no medical insurance policy number. 2. Child #2 - No name, address and telephone number for 1 of 2 emergency contacts, and no signed liability insurance form.
