The following inspection took place between the hours of 10:15 am through 11:30. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this unannounced monitoring inspection there were a total of 10 children (13 points) with 2 caregivers. A total of 3 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: morning circle, learning about planets, yoga, Montessori activities that included; math, phonics, and words that rhyme. Standard #: 22VAC40-111-180-A Description: Based on record review, the assistant does not have a current report of Tuberculosis screening every two years from the date of the last screening. Evidence: Caregiver #1 does not have a current TB screening, the last screening is dated 1/29/2015, not within every two years. Plan of Correction: He will get the updated screening and place the report in the record.
