63.2 Liability Insurance Disclosure
An unannounced monitoring inspection was conducted on Monday, May 1, 2017, from approximately 2:35pm until 5:00pm. Present today were ten children totaling twenty-eight points with three caregivers. All three caregivers are currently certified in first aid and CPR. The children were observed during naptime, diapering, snack and during free play. Lunch: Ham, green peas, apples, bread and milk PM Snack: Vanilla wafers, yogurt, juice The last practice evacuation drill was conducted on 4/26/17. Three children's records were inspected.
Standard #: 22VAC40-111-60-A Description: Based on inspection of children's records, the provider did not ensure each record contained all required information. Evidence: Out of three children's records inspected, Child #2 did not have the address of their second emergency contact. Plan of Correction: Per Provider: I will get that information today.
Standard #: 22VAC40-111-90-A Description: Based on inspection of children's records, the provider did not ensure each child's records contained documentation of immunizations before the child attends the family day home. Evidence: Out of three children's records inspected, Child #1, began attending 4/6/17 and Child #2, began attending 4/10/17. Neither child's record contains documentation of immunizations. Plan of Correction: Per Provider: I will have both families contact their doctors for a copy of each child's immunizations.
