54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted from 2:10 p.m. to 4:10 p.m. At the time of entrance, 9 children, ages 11 months to 3 years old, for a total of 24 points were in care with the provider and an assisitant. The records sample size consisted of 4 children?s records and 3 provider / assistant/ household member records. Children were observed resting quietly at nap time, having an afternoon snack of fresh strawberries and cinnamon bread, and watching a children's program on the television. Restroom and diapering procedures, hand washing and table sanitizing were also observed. Medication administration was not observed. The provider states that no children are currently administered medication while in care, there have been no accidents or injuries incurred by children in care requiring first aid or medical treatment and children are not routinely transported in a vehicle while in care. Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, the licensee did not ensure that each enrolled child's record contains the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. Evidence: Four children's records were reviewed. The record for child #4 contained only one of two required emergency contacts to be called in an emergency if the parents cannot be reached. Plan of Correction: The licensee will speak with the parents today and ask that a second emergency contact person be added to the record by the end of the week.
Standard #: 22VAC40-111-70-A Description: Based on a review of children's records, the licensee did not ensure that before the child's first day of attendance, every child's parents were provided in writing the current names of the adult assistant or substitute provider(s) and complete provisions of the family day home's emergency preparedness and response plan. Evidence: Four children's records were reviewed. 1.) The record for child #1 did not contain information regarding the family day home's emergency plan. The records for children #2, #3 and #4 did not list the location where the family day home would relocate in the event of an emergency requiring evacuation from the home 2.) The records for children #1, #2, #3 and #4 contain information that caregiver #3 and household member #2 may provide care for children up to 30% of the time each week if the Licensee needs to be away from the home. However, in discussing training requirements with the Licensee she stated that household member #2 no longer serves as an assistant caregiver. This change in arrangements should be updated in the records and reviewed with parents. If household member #2 will serve as an assistant, then the required 16 hours of annual training would need to be completed. Plan of Correction: The licensee stated that she would update this information in the records and review the updates with parents this week.
