- Technical Assistance:
Technical assistance today included storage of infant seats, required records before a child can begin care, placing infants on back to sleep -- and infants who turn themselves.
An unannounced renewal inspection was conducted today from 10:30 - 12:40 pm. There were 12 children in care with the provider and two assistants for 38 points. I observed children at play with toys, learning to count in Spanish and English; singing; preparing for lunch (homemade tiny fishcakes, rice and veggies) and naptime. I also observed staff's calm and efficient handling of a situation that arose when a child seemed to develop a reaction to an allergen. If you have any questions, please contact me at firstname.lastname@example.org
Standard #: 22VAC40-111-60-A Description: During today's inspection, a new child in care did not have a complete record accessible. Evidence: Child # 1, who began care today, had only a physical exam and immunization form available today. Plan of Correction: His mother took the forms and will bring them later.
Standard #: 22VAC40-111-180-A Description: During a review of records today and through staff confirmation, one household member did not have a current tuberculosis screening. Evidence: The TB screening for Household Member # 1 was expired 1/16. Plan of Correction: He is waiting for a copy of the report from the doctor. I will mail it to you.
Standard #: 22VAC40-111-210-A Description: During a review of records today and through staff confirmation, it was found that there were not the number of required training hours (16 hours annualy) for each assistant. Evidence: Assistant 1 & 2 had 2 hours of training this year. Plan of Correction: Staff will work on the on-line classes.
Standard #: 22VAC40-111-240-A Description: Some areas of the family day homes were observed to not be maintained in a safe condition. Evidence: On the glass doors that lead to the backyard, the rubber sealant for the door was observed to be coming off, leaving a loop accessible at child height. There were sharp areas along the bottom of the doors where a panel was coming off, exposing some sharp edges. There was also a loose brick at the corner of the home, accessible at child height. An open ladder was accessible today on the deck where children play. Indoors: There was an area of peeling paint in the children's room. Plan of Correction: The seal will be cut off. I have called for a handyman or contractor to do work. In the meantime, we will tape over the sharp areas. The brick will be cemented in. The ladder will be put away. The peeling paint in the classroom came from where something on the wall was taken off. We will cover it up so it is not accessible.
Standard #: 22VAC40-111-280 Description: During the inspection, not all hazardous items were observed to be locked as required. Evidence: Under the kitchen sink, a cabinet without a lock or latch was where household cleaners were located today. Plan of Correction: We will get a latch.
Standard #: 22VAC40-111-480-D Description: During today's inspection,a piece of equipment was observed not to be used in accordance with manufacturer's instructions. Evidence: A crib was observed today with two mattresses inside. This is not how the manufacturer intended the crib to be used. Plan of Correction: The mattress will be removed.
