The renewal forms can be found on the departments web site www.dss.virginia.gov, look for current providers(including form) and you will find the renewal there.
The provider is present with 4 children during this monitoring inspection. A total of 13 points. The children are getting ready for nap and nap time was observed. The children had rice, beans and cheese, bananas and milk was also offered for lunch today. The provider has a set up with many different interest areas and room for both self directed play and teacher directed play. It is Monday so some of the children are having a hard time settling for nap , the provider is rocking them and rubbing backs to get everyone to sleep.
