Verbally reviewed: - allergies, sensitivities and dietary restrictions (posting, plans, preparation) - homeless children - emergency closure and reporting of significant injuries - transportation
Licensing Inspector arrived at 11:00 AM and departed at 1:00 PM to conduct a subsidy health & safety inspection in conjunction with an unannounced renewal. The sample size was three caregiver and three children's records. Children were observed reading books, eating lunch and napping. Violations were noted in the areas of caregiver qualifications & training and special care provisions & emergencies.
Standard #: VENDHOM-000-012-A Description: Based on review of the caregiver's record, it was determined that the vendor did not complete Virginia Pre-Service Training for Child Care Staff sponsored by the Department of Social Services. Evidence: 1. There was no certificate to verify that the vendor had completed Virginia Pre-Service Training for Child Care Staff sponsored by the Department of Social Services. 2. Staff #1 (vendor) stated that she had not completed the training. Plan of Correction: Training will be completed and certificate placed in the record.
Standard #: VENDHOM-000-012-B Description: Based on review of the caregiver records, it was determined that the caregivers did not complete Virginia Pre-Service Training for Child Care Staff sponsored by the Department of Social Services within the first 90 days. Evidence: 1. There were no certificates to verify that the assistant caregivers had completed Virginia Pre-Service Training for Child Care Staff sponsored by the Department of Social Services. 2. Staff #1 (vendor) stated that they had not completed the training. Plan of Correction: Training will be completed and certificate placed in the record.
Standard #: VENDHOM-000-029-B Description: Based on review of the home's emergency plan, it was determined that the emergency preparedness plan did not contain all required elements. Evidence: The emergency preparedness plan did not contain information for lockdown and reunification of children with parents. Plan of Correction: Provider will update emergency preparedness plan to include all required elements.
