An unannounced monitoring inspection was conducted with provider and assistant. There were ten day care children present, ranging in age from 11 months to 4 years, totalling 32 points. The children were having lunch, reading books, playing with age appropriate toys, and preparing for rest. Three children's files and five household/assistant files were reviewed - please see Supplemental Summary. There are no children receiving medication at day care today. Provider is current in Medication Administration Training until 1/25/2018. First aid and CPR certification was renewed 3/18/2017. Standard #: 22VAC40-111-60-B Description: Based on review of three new children's records, the facility failed to have all information required in the file. Evidence: Child C did not have documentation of a second emergency contact name, address, and phone number. Plan of Correction: Provider will request the information from the parents again.
Standard #: 22VAC40-111-250-C Description: Based on observation, the provider failed to remove items hung from the sides of cribs once the children begin to push up or turn five months old. Evidence: one crib had a mobile attached on the side of the crib; another crib had a stuffed Eeyore toy hung from the side of the crib. The youngest infant in care is 11 months old. Plan of Correction: Provider will remove items from the cribs once the children turn 5 months old or when the child begins to push up on hands and knees.
