An unannounced monitoring inspection was conducted this date. The inspection was conducted between 10:30am and 12:15pm. Provider and 2 staff present with 11 children in care today (30 points). Observation of circle time which included book reading, free play which promoted both fine and large motor skills. Staff were observed diapering, hand washing and bottle feeding children. A smooth transition from free play to lunch was observed and hand washing afterwards. Observation of physical plant was made which included the large area for programming, area for food preparation and the room designated for cribs. Positive interaction was observed between children and staff. Staff presented knowledgeable of the children's individual needs. Ample supplies, toys and equipment on site which were determined appropriate for the age of the children in care. Thank you for your cooperation. Standard #: 22VAC40-111-100-A Description: Based on review of children's records, one child record was incomplete. Evidence: Child #1 began care on 1/3/2017. A physical has not been obtained to date. Plan of Correction: Provider will follow up with parent to ensure that a physical is obtained.
