Technical Assistance:
Consultation was given on submitting renewal study application as the Conditional license expires in early June. As of the day of the inspection, no renewal application has been received. In addition, a general permission for walking field trips should be completed by parents as the provider states that the children walk to the local park about a block away. The model forms were given to the provider for general field trips as well as topical cream authorizations.

Comments:
An unannounced renewal study was conducted from 10:00-11:00. Five children were present for sixteen points with two caregivers which is within ratios for supervision. Children were observed in bottle feeding, free choice and preparation to go outside in the back common space to play. Home is well maintained and equipped with materials for young children. Emergency preparation is current, attendance was accurate and bottles were labeled. One newly enrolled child's enrollment file was reviewed and up to date. No medications were reported to be on-site today. All food for children is bought from home. Please sign and date the report and return it to me within 5 calendar days from date of receipt.
