Discussed the following with the Provider: - TB testing and procedures if testing is positive. - Location of the Emergency Preparedness Plan.
An announced inital inspection began today at 11:15am. The physical plant, 4 caregiver/household member files, 4 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today. There were 4 children, totaling 10 points, under the supervision of 2 caregivers. During the inspection, children were observed enjoying free play, listening and singing to music, and working with shapes. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served grilled cheese sandwiches, cucumber, grapes and milk. The Provider is not certified in MAT (medication administration training) and does not plan to administer medications at the FDH. Standard #: 22VAC40-111-100-A Description: Based on file review, 1 out of 4 children's records does not have documentation of a physical examination within 30 days after the first day of attendance. Evidence 1. Child #4 began care on 1/3/17. There is no physical examination on file. Plan of Correction: The Provider thought she had it. She will continue to look for it. If it is not found, she will ask the parents for a copy.
Standard #: 22VAC40-111-120-B Description: Based on file review, 1 caregiver's record does not have all required information. Evidence 1. Caregiver #2 does not have documentation that two or more references as to character and reputation as well as competency were checked before employment. Plan of Correction: The Provider called two references, but did not document this. She will complete the documentation and keep it on file.
Standard #: 22VAC40-111-90-A Description: Based on file review, 1 out of 4 children's records does not have documentation of an immunization record prior to receiving care at the FDH. Evidence 1. Child #4 does not have an immunization record on file. Plan of Correction: The Provider thought she had it. She will continue to look for it. If it is not found, she will ask the parents for a copy.
Standard #: 22VAC40-111-90-C Description: Based on file review, 2 out of 4 children's records do not have additional immunizations every six months up to the age of 2 years. Evidence 1. Child #2 is 21 months old. The most recent immunization record on file was completed when the child was 2 months old. 2. Child #3 is 11 months old. The most recent immunization record on file was completed when the child was 2 months old. Plan of Correction: The Provider will ask the parents for this.
