Comments:
The licensing inspector conducted an unannounced subsidy health and safety inspection from 10:00am - 1:00pm. There were seven children in care for a total of seventeen points with three caregivers. The children were observed playing with the available toys, watching cartoons and during lunch. Three caregivers and two children's records were reviewed. Areas of non-compliance are identified on the violation notice. If you have any questions, please contact your licensing inspector at (276) 608-3758. Thank you for your cooperation.

Violations:
Standard #: VENDHOM-000-012-C
Description: Based on record reviews, the family day home failed to have caregivers provide documentation of completion of the preservice training. Evidence: 1. The inspector reviewed three caregiver's records. There were no certificates in the three caregiver records confirming that they had completed the ten hours of preservice training.

Plan of Correction: All three caregivers completed the preservice training at the end of 2016, but the certificates have been misplaced. If the certificates cannot be found, all three caregivers will repeat the training. The training certificates will be sent to the licensing office.
