An exit interview was conducted with the provider to discuss the results of today's inspection. Consultation was given regarding parent notifications and allergy requirements.
An unannounced Subsidy Health & Safety inspection was conducted today, May 19, 2017 to determine the family day home's compliance with subsidy vendor requirements. The inspection was initiated at 11:29am and concluded at approximately 1:30pm. All subsidy requirements were reviewed and compliance determined. Administrative records, rooms used by children, and outdoor play areas were inspected. See attached notice for violations found during this inspection.

Standard #: VENDHOM-000-012-B Description: Based on the inspection of administrative records, the provider has not completed the Virginia Preservice Training within 90 days of service. Evidence: During the inspection of administrative records, the provider did not complete the required Virginia Preservice Training within 90 days of service. The provider was taking the incorrect course on the website, which can be used for annual training; however, it does not substitute for the Virginia Preservice Training. Plan of Correction: Per provider: I accidently took the wrong the course and will immediately take the correct training. My licensing inspector showed me the correct course to take on the website.
Standard #: VENDHOM-000-029-B-5-F Description: Based on the inspection of the family day home's lockdown plan, the plan did not include procedures to reunite children with a parent or authorized person designated by the parent to pickup the children. Evidence: During the inspection of the lockdown plan, the written procedures for reunification was not listed. Subsidy regulations require written procedures to address reuniting children with parents or authorized person by the parent to pickup the child. Plan of Correction: Per provider: I will immediately complete the reunification section in my lockdown plan.
