COMPLAINT # 47492 A joint investigation was conducted today at the home. There were 6 children present with the provider and assistant for 19 points. Records were reviewed and interviews were conducted. The following standards were examined: 630: Behavior guidance 640: Forbidden actions Due to a lack of a preponderance of evidence, these standards were found to be in compliance. However, the following standards were not part of the original complaint but found to be in violation during the course of the investigation. 130 B: Caregiver's ability to read, write and speak in English as required by the standards 480 B: Craft materials not age appropriate for the age of children in care. 510 B: Child sleeping at nap without individual cot. Description: During today's inspection, it was determined that Assistant # 1 did not meet the general qualifications for caregivers. Evidence: Assistant # 1 was not able to speak, read and write in English as required for the position.

Plan of Correction: She will take English classes.

Description: Not part of the original complaint, but found during the investigation- Some materials used by children for a recent art project were not appropriate to the age of children in care. Evidence: Pictures hanging on the wall that were made by children featured tiny glued button eyes and a metal bell charm that could pose a choking hazard.

Plan of Correction: Removed from the wall during inspection.

Description: Not part of the original complaint, but observed during the investigation: During today's inspection, one child was observed to be sleeping in an area without an individual cot or bed. Evidence: Child # 2 was observed sleeping on the platform in front of the unused fireplace during naptime today.

Plan of Correction: She likes to sleep there, but we will put down a cot for her.
