Consultation was given on model forms for topical creams, dating and signing birth certificate documentation, playing outside, on-line training resources and logging substitute hours if the provider is away from the home more than two hours each day.
An unannounced subsidy visit was conducted today from 9:30-11:30. Two children were present with two caregivers for six points. Home has dedicated space for day care that is well equipped with toys and materials. A separate sleeping area is available. The provider has Medication Administration Training (MAT) certification and one child enrolled requires emergency medication in case of an allergen exposure. Lunch today will be chicken and rice, carrots, fruit and milk. All emergency supplies are on-site and drills are up to date. Children records for two kids, provider and assistant caregiver records as well as other household members were reviewed and complete. Areas of non-compliance are identified in this report. Standard #: VENDHOM-000-006-B Description: Based on record review, one child did not have a written allergy plan Evidence: Child #1 has a documented allergy and a plan is not present for provider reference. Plan of Correction: Parent is bringing in medication and authorizations to complete plan.
Standard #: VENDHOM-000-012-C Description: Based on provider statement, assistant has not completed the VA Pre-service training within 90 days of signing vendor agreement. Evidence: Assistant does not have 10 hours of pre-service training as required by April 2017. Plan of Correction: Completed training and documentation e-mailed to Licensing Inspector.
