Technical Assistance:
National fingerprinting will be required for all persons over the age of 18 residing in the home. Further information will be disseminated over e-mail in the next few weeks.

An unannounced renewal inspection was conducted with provider. There were three day care children and three residential children present totalling 15 points. The day care children are 17 months, 17 months, and 4 years old. The resident children are 17 months, age 4, and age 3. Three childrens files and four household files were reviewed - see Supplemental Summary for missing items. The provider's first aid and CPR is valid until 1/9/2018. The children were settling down for rest time. Time of today's inspection: 12:30 p.m. to 2:00 p.m.

Standard #: 22VAC40-111-60-B
Description: Based on review of three children's records, the provider failed to have all required information in each child's file. Evidence: Child A and Child B did not have the home address or home phone number for the custodial guardian. Child A and B did not have the name address, and phone number of custodial guardian's place of employment. Child B did not have the name and policy number of the child's medical insurance.

Plan of Correction: Provider will request the information from the custodial guardian.

Standard #: 22VAC40-111-170-A
Description: Based on review of four household files, the provider failed to have a tuberculosis test or screening for each adult household member prior to coming into contact with children. Evidence: Household member 3 did not have documentation of a tuberculosis test or screening.

Plan of Correction: Household member 3 was going to have the tuberculosis test read today.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on review of four household records, the provider failed to have a repeat search of the central registry (CPS check) for all household members. Evidence: Household member 2 did not have documentation of a repeat search of the central registry (expired 5/9/2017).

Plan of Correction: Household member 2 has a repeat search of the central registry, but left it at the employer. The CPS check will be picked up from the day care.
