An unannounced renewal study was conducted from 9:30 a.m.-11:00 a.m. with the provider. Four children, ages 2 years, 2 years, 3 years, and 4 years old, were under the care and supervision of two caregivers for a total of eight points. Children were observed playing with baby dolls, building with Duplos, and playing the "Hungry, Hungry, Hippos" game. Required information was posted. A complete first aid kit, flashlights, and weatherband radio were present. The last emergency escape drill was 5/4/17. Both caregivers hold current certification in first aid and CPR. Two medications were reviewed and in compliance. The provider holds current medication administration training (MAT) certification. Four household members' records and four children's records were all reviewed and complete. Description: Based on a review of four children's records, the provider did not obtain documentation of immunizations for a child prior to the child's first attendance. Evidence: Child A's (date of first attendance: 8/4/13) immunizations were dated 9/16/13.

Plan of Correction: Child A had moved to Virginia during this time. His parent had to wait on his medical records being mailed, which caused this delay.
