An annual subsidy health and safety inspection (SHSI) was conducted from 9:30 a.m.-11:00 a.m. with the provider. Four household members' records, four children's records, and two medications were reviewed. Based on observation and documentation review, two violations of the subsidy requirements was issued. Description: Based on observation and vendor interview, the vendor did not post or keep immediately accessible a current list of all children's allergies, sensitivities, and dietary restrictions. Evidence: There was no posted or accessible allergy list.

Plan of Correction: The vendor created and posted a list of allergies during the inspection.

Description: Based on a review of four children's records, the vendor did not maintain complete records. Evidence: Child A's record did not include a written care plan for a diagnosed food allergy to peanuts and tree nuts.
