Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted today. Observed 10 children (24 points) directly supervised by 2 caregivers. The children were observed during rest time. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Inspection conducted from approximately 12:30pm to 1:10pm. Description: Based upon observations made, the vendor did not post, with parental approval, or keep in a way that is immediately accessible to a caregiver, a current list of all children's allergies, sensitivities, and dietary restrictions. Evidence: A list with Child A's allergy was not posted.

Description: Based upon documentation reviewed, a child's record was incomplete. Evidence: Child A has a diagnosed food allergy; however, a written allergy plan was not part of the record.

Description: Based upon documentation reviewed, all caregivers did not complete the Virginia Pre-Service Training for Child Care Staff within the first 90 days of service. Evidence: One of the two caregiver records reviewed did not documentation to verify the Pre-Service Training for Child Care Staff was completed within the allowable time frame (Caregiver #1).
