54.1-3408 Provider must be MAT certified to administer prescription medication.
Today an unannounced monitoring inspection was conducted with Provider. This visit took place in the morning between 9am and 10:50am . There were 12 children present with the Provider and 2 assistant(s) for a total of 30 points within the supervision guidelines. The children were observed completing breakfast which consisted of cheerios, milk and fresh strawberries. Hand washing was observed after breakfast and a nice transition to downstairs to free play was observed. Children were playing in small groups and individually. Diapering observed with one child. Positive interactions were observed during this inspection. A complete inspection of the area used for programming, children, household and staff records, fire drill log, menu, attendance and emergency procedures were reviewed during this inspection. Standard #: 22VAC40-111-780-B Description: Based on child record review, annual review of emergency contact information was not completed. Evidence: Child #4 and #5's emergency contact information was not reviewed on an annual basis. Plan of Correction: I will have all parents review the emergency contact information on file today.
Standard #: 22VAC40-111-830-A Description: Based on review of emergency evacuation documented drills, monthly drills have not been conducted. Evidence: No documentation of completed evacuation for the months of Feb, March, April and May 2017. Plan of Correction: I will conduct a fire drill this month and will make sure fire drills are held and documented every month.
Standard #: 22VAC40-111-60-B Description: Based on child record review, one child record was incomplete. Evidence: Child #3's record contains only 1 of the 2 required emergency contacts. Plan of Correction: I will obtain the second emergency contact today from the parent.
