22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced monitoring inspection was conducted in the Family Day Home (FDH) on June 12, 2017. There were 4 children present, the points totaled nine (9). Reportedly, eight children are currently enrolled. Three adult caregivers were providing supervision and assisting with the children's care. The children were observed by the LI during the morning program. The observed learning activities included provider-directed and child-initiated. A caregiver led the children during free play, sang several songs with the children, the children also played with music instruments, and listened to a story. Other observed activities included diapering, hand washing procedures, the lunch meal, and nap/rest time. The caregivers were engaged with the children. A sample of 2 children's and one complete caregiver record was reviewed. A discussion was held with the Provider regarding subsidy requirements and future changes in the USDA meal pattern. As reported the FDH does not administer medications. Findings of the inspection were reviewed with the Provider at the exit. The inspection was conducted from approximately 9:30am to 12:15pm. Standard #: 22VAC40-111-880 Description: Meals and snacks provided by the family day home shall follow the most recent, age appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). Evidence: The LI observed the children during the lunch meal, milk was not served with the meal, apple juice was served to the children. Plan of Correction: The Provider will serve milk with each meal.
Standard #: 22VAC40-111-60-B Description: Based on a sample review of two children's records, the Provider failed to ensure required information was complete in each record. Evidence: The LI reviewed two children's records, information was not complete for the following: child 1. the record did not identify the name, address, and telephone number of each custodial parent's place of employment; child 2. the insurance policy number was not complete and the address and telephone number of the custodial parent's place of employment was not complete. Plan of Correction: The Provider will review the records with each child's parent to ensure information is complete.
Standard #: 22VAC40-111-180-A Description: Based on records reviewed, the Provider failed to obtain a current report of tuberculosis screening form in accordance with the requirements in 22 VAC40-111-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department. Evidence: The LI reviewed records of household members and caregivers, three of three TB screenings had expired as of May, 2017. Plan of Correction: An appointment will be scheduled to obtain updated TB screenings for each individual.
Standard #: 22VAC40-111-680-A Description: Based on direct observations, a caregiver failed to wash their hands with liquid soap and warm running water after diapering a child or assisting with toileting. Evidence: The LI observed a caregiver during the diapering procedure for 3 children back to back. The caregiver put new gloves on after each diaper change but failed to wash hands after each change and prior to changing gloves. The caregiver used gloves that were designed for household chores. During a diaper change the gloves were torn by a child. Plan of Correction: All caregivers will wash their hands at the bathroom sink with soap and water after each diaper change or after assisting a child with toileting.
Standard #: 22VAC40-111-680-B Description: Based on direct observations, caregivers failed to ensure that children's hands were washed with liquid soap and warm running water before eating and after toileting or diapering. Evidence: 1. A child's hands were not washed prior to eating lunch. Child #1. was seated in a high chair and a caregiver fed lunch to the child with a spoon. 2. The LI observed diapering procedures for 3 children, the caregiver did not direct the children to wash their hands after the diapering procedure. Plan of Correction: All caregivers will instruct the children to always wash their hands before eating and after diapering or toileting.
Standard #: 22VAC40-111-690-D Description: Based on direct observations, a caregiver failed to ensure the diapering surface was cleaned and sanitized after each use. Evidence: The LI observed a caregiver during diapering of 3 children. The diapering surface was wiped with a disinfectant wipe after each change. The surface was not cleaned with soap and water and sprayed with a sanitize agent and allowed to air dry. Plan of Correction: Soap and water will be used to clean the diapering surface and a mixed bleach/water solution will be sprayed on the surface and air dried after each use.
