Discussed with the provider: Subsidy enrollment. You will call my office to get the email address to contact the main Subsidy office.

Comments:
An unannounced monitoring inspection was conducted beginning at 10:00 and ending at 11;10. There were 6 children in care, including two who live in the home. Reviewed the files for 2 newly enrolled children - see violation notice Emergency escape drills and shelter in place drills are being done monthly. Violations:
Standard #: 22VAC40-111-50-A
Description: Based on observation and interview with the provider, it was determined that the provider is not keeping a daily attendance sheet.

Plan of Correction: The provider will use the attendance sheet and assure that it is completed daily.

Standard #: 22VAC40-111-60-B
Description: Based on a review of the files for two newly enrolled children, it was determined that the files were not complete. Evidence: The files for Child #1 and #2 were missing medical insurance # and a phone number for the second emergency contact.

Plan of Correction: The provider will get the information from the parent.
