Discussion was held on the topics of current enrollment, current staffing (Staff #2 is being used as a lead when needed), and the status of a family visitor and what documentation may be required.
The following inspection took place between the hours of 10:30 am through 1 pm. Today, I met with the Provider on site at her Family Day Home (FDH) located in Broadlands, Virginia. At the start of this unannounced 2 month follow-up inspection there were a total of 6 children (1 was a resident) (12 points) with 1 caregiver. A total of 12 children?s files and 5 staff/household member?s files were reviewed. The children were engaged in the following: freeplay, toys, games, outside play with a water table in the FDH's backyard, bathroom breaks, hand washing, lunch, and other organized activities. Standard #: 22VAC40-111-60-B Description: Based on review, 2 of 12 children's files reviewed indicated that not all of the required information was contained within each child's record. Evidence - 1. On the date of inspection (06/14/2017), the following children's files were not complete: Child #7, and Child #8. 2. The addresses for both listed emergency contacts were not available for Child #7. The insurance policy number was not available for Child #8. Plan of Correction: All missing information shall be obtained and placed in the applicable child's file.
Standard #: 22VAC40-111-90-A Description: Based on review, 2 of 12 children's files reviewed indicated that before a child may attend the Family Day Home (FDH), the Provider did not obtain documentation that the child had been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations. Evidence - 1. On the date of inspection (06/14/2017), the following children were without an immunization page: Child #7, and Child #8. 2. The start date for Child #7 was listed as 06/13/2017, and the start date for Child #8 was listed as 05/22/2017. Plan of Correction: All missing immunization pages shall be obtained and placed in the applicable child's file.
Standard #: 63.2-1809.1-B Description: Based on review, 1 of 12 children's files reviewed indicated that documentation of the proof of liability was acknowledged was not available for all children. Evidence - 1. On the date of inspection (06/14/2017), an acknowledgement that the proof of liability was reviewed by the Parent of Child #7 could not be determined. 2. This documentation was not on file. Plan of Correction: This documentation shall be obtained and placed on file.
