22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced monitoring inspection was conducted today, June 15, 2017 from approximately 10:30 am to 2:30 pm. The provider participated in the inspection. During the inspection, seven children with a value of 17 points were observed with the provider and two additional caregivers. Children were observed in circle time singing songs. Children also had time to play with toy blocks, listen to stories and time playing outside with small riding toys. Later lunch time and nap time were observed. Hand washing was conducted before and after the meal. Today's lunch consisted of chicken, mac & cheese, applesauce, green beans and milk. Caregivers interacted with children in a positive manner. All areas of the family day home were inspected. The provider had all required information posted to include the license, last inspection notice, emergency numbers, evacuation maps, menu, allergy information and attendance sheets. The family day home was equipped with age and stage appropriate materials and supplies. The first aid kit and non-medical emergency supplies were inspected and found complete for the home and van.
Three caregiver records were reviewed along with seven children's records.
One caregiver has current MAT certification. Two caregivers have current CPR & First aid training.
Currently there are no children with medications.
The most recent evacuation drill was documented on 05/17/2017. The two required shelter-in-place drills for the 2016 year were completed on 08/16/2016 and 12/23/2016.
A new crib was reviewed and met federal safety standards. (02/2017)
The emergency preparedness plan was reviewed and updated 12/19/2016.
Violations from the previous inspection on December 15, 2016 were checked for corrections. There were no repeat violations cited.

Standard #: 22VAC40-111-120-B Description: Based on review of care giver records, the provider did not obtain documentation that two or more references as to character and reputation as well as competency were checked before employment. Evidence: The record of care giver #3, hired 05/02/2017 did not have documentation of a second reference. Plan of Correction: Per the provider: I will get the second reference for the care giver.
Standard #: 22VAC40-111-140-B-1 Description: Based on review of the substitute providers record, the provider did not obtain documentation of a high school program completion or the equivalent for the substitute provider. Evidence: The record of care giver #2, identified as the substitute provider, did not have documentation of a high school program completion or the equivalent on file. Plan of Correction: Per the Provider: I will have the care giver provide a copy of her high school diploma.
Standard #: 22VAC40-111-320-H Description: Based on observations, the provider did not ensure all flammable and combustible materials were stored in an area inaccessible to children. Evidence: Two canisters of gas for the lawn mower were observed in a unlocked shed in the rear yard where children in care were playing. The two canisters were approximately two feet from the ground and within reach to children in care. The shed door was cracked and not closed securely. Plan of Correction: Per the Provider: I will move the cans of gas to the other shed and lock the door. I plan on taking down the little shed.
Standard #: 22VAC40-191-60-C-1 Description: Based on review of care giver records, the provider did not obtain an original criminal history record report within 30 days of employment. Evidence: Care giver #2, hired 01/03/2017, had an original criminal history record report that was dated 02/21/2017, exceeding the 30 days from hire allowance to obtain. Plan of Correction: Per the provider: will mail off request for background checks within seven days of hire to make sure they are returned on time.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of care giver records, the provider did not obtain a central registry finding within 30 days of employment. Evidence: Care giver #2, hired 01/03/2017, had a central registry finding that was dated 04/03/2017, exceeding the 30 days from hire allowance to obtain. Plan of Correction: Per the provider: will mail off request for background checks within seven days of hire to make sure they are returned on time.
