The Licensing Inspector for Michelle Borst family day home conducted an unannounced monitor inspection on 6/15/2017. Inspection started at 2:10 PM. Records for the 3 enrolled children were reviewed. Provider does not administer medication and does not transport children. Staff/household records were reviewed during the inspection. An exit interview including review of Risk Assessment & areas of non-compliance found were reviewed with the Provider on 6-15-2017. Inspection concluded at 4:00 PM. Standard #: 22VAC40-111-830-A Description: Based on review of emergency documents, the Licensee failed to have monthly evacuations drills. EVIDENCE: Evacuation drill was not conducted for May 2017. Plan of Correction: Will conduct drill
Standard #: 22VAC40-111-50-A Description: Based on inspection & discussion with the Provider, the Licensee failed to keep a written record of children in attendance each day. EVIDENCE: There are 4 enrolled children; documentation of daily attendance was unavailable. Provider advised written daily attendance has not been completed since April 2017. Plan of Correction: Provider will start back doing it.
Standard #: 22VAC40-111-140-B-3 Description: Based on record review and discussion with the Provider, the Licensee failed to have current first aid & cpr certification. EVIDENCE: The Provider did not renew the required first aid & cpr training. Most recent certification expired 2/10/2017 Plan of Correction: Provider will schedule appointment
Standard #: 22VAC40-191-40-D-1-c Description: Based on record review & discussion with the Provider, the Licensee failed to update Background Checks (Criminal Record Check & Central Registry) EVIDENCE: Staff #1 Background Checks have not been updated before three years since the most recent Background Checks. Criminal Record & Child Protective Services Backgrounds checks were not updated. Most recent CRC was completed on 2/25/2014 and expired 2/25/2017.
