Comments:
An unannounced monitoring inspection was conducted on June 13, 2017 from approximately 11:30 AM to 2:00 PM. Provider and one assistant were present with 12 children ( 26 points). Children were observed playing outside, building towers, eating lunch and napping . Three children records and two caregiver records were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, diapering procedures, hand washing procedures, nutrition, children and caregiver records, emergency evacuation and shelter-in-place drill documentation, emergency supplies and required postings. Exit interview with the provider. Exit interview was conducted with the provider.

Violations:
Standard #: 22VAC40-111-770
Description: Based on observation , the provider did not ensure there was a working battery operated weather band radio. Evidence: During the inspection conducted on June 13, 2017, the provider did not have working battery -operated weather band radio and extra batteries.

Plan of Correction: The provider will replace the batteries today.

Standard #: 22VAC40-111-920
Description: Based on observation, provider did not ensure a menu for the current one-week period is posted. Evidence: During the inspection conducted on June 13, 2017, the menu was not posted for the week of June 12-June 16. The menu the provider did not have completed for the week.

Plan of Correction: The provider was completing the menu during the inspection.

Standard #: 22VAC40-111-140-B-3
Description: Based on record review, the provider did not have a current certification in cardiopulmonary resuscitation (CPR) with in the past two years. Evidence: During the inspection conducted on June 13, 2017, the provider CPR and First Aid expired February 2017. The provider confirmed there was no one present with CPR and First Aid .

Plan of Correction: The provider and staff will be scheduling a CPR and First Aid training. The provider has contacted instructor.

Standard #: 22VAC40-111-240-A
Description: Based on observation, provider did not ensure areas and furnishings of the family day home, inside and outside, are maintained in a safe and operable manner. Evidence: During the inspection conducted on June 13, 2017, outside the following was observed: -On the front deck there were two protruding nail on the guard railing. - The fence in the front had multiple broken slats caused sharp edges and their were nails protruding . - The back fence had a panel detached and it was leaning. - On the back deck the gate that opens and closes is detaching and exposing nails.

Plan of Correction: Provider was already working on replacing the fencing and fixing the fencing in the back.

Standard #: 22VAC40-111-280
Description: Based on observation, the provider did not ensure potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates shall be stored away from food in areas inaccessible to children. Evidence: During the inspection conducted on June 13, 2017, the provider had a glass cleaner in the kitchen on the counter and bleach in water stored on shelf in the living room. There were children present from infant to school age. The items were reachable for school age children.

Plan of Correction: The provider will remove bleach and water and glass cleaner.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the provider did not ensure the finding of the most recent inspection of the facility was posted. Evidence: During the inspection conducted on July 13, 2017, the licensing inspectors observed the findings of the most recent inspection were not posted .

Plan of Correction: The provider will look for the violation notice.
