An unannounced full renewal inspection was conducted today. The provider and three assistant caregivers were caring for 5 children, totaling 11 points. The children were observed playing with toys, building with blocks, and interacting with each caregiver. This inspection was completed between approximately 10:15 am and 11:35 am. Standard #: 22VAC40-111-180-A
Description: Based on a review of caregiver records, it was determined that three required repeat TB Health Screening reports have expired. Evidence: The TB Health Screening for the provider and two assistant caregivers expired in February, 2016.

Plan of Correction: We will all get a new TB screening done.

Standard #: 22VAC40-111-480-D
Description: Based on observations, it was determined that the provider was not using one piece of equipment in accordance with the manufacturer's instructions. Evidence: A portable high chair seat was in place on the floor. It was reported to the Inspector that at times one child sits in this portable seat on the floor, however the attached instructions indicate that this chair is to be used only when securely attached to a full size chair.

Plan of Correction: We will begin having that child sit in a regular high chair.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on a review of records, it was determined that one required background check for a household member has not been completed. Evidence: In December, 2016 one household member came of age to have a Child Protective Services Registry (CPS) check completed. The provider did not submit a CPS background check request for this person.

Plan of Correction: The CPS check request will be submitted.
