Technical Assistance:
The provider reported to the Inspector that she is no longer a subsidy provider.

An unannounced full renewal inspection was conducted today. The provider and two assistant caregivers were caring for 7 children, totaling 24 points. The children were observed playing with toys, eating lunch, and taking their afternoon nap. The children were served baked chicken, whole wheat bread, bananas, red beans, and milk for lunch today. Standard #: 22VAC40-111-830-B
Description: Based on a review of administrative documents, it was determined that the provider did not conduct at least 2 shelter-in-place drills during the past year. Evidence: The provider did not practice any shelter-in-place drills in 2016.

Plan of Correction: I will do at least 2 drills this year.

Standard #: 22VAC40-111-900-B
Description: Based on observations, it was determined that two children were allowed to lie down with sippy cups. Evidence: Two children were observed lying on their cots with sippy cups at nap time.

Plan of Correction: We will no longer allow the children to drink on their cots.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: 1. Child record #1 does not include the parent's medical insurance information. 2. Child record #4 does not include: the work address for each parent; the parent's medical insurance information; the provider's signature indicating that she viewed the child's original proof of identification.

Plan of Correction: The records will be updated and complete.

Standard #: 22VAC40-111-240-A
Description: Based on observations, it was determined that one piece of equipment has not been maintained as required. Evidence: There is a small rip in the covering of the diaper changing pad.

Plan of Correction: I will get a new changing pad.

Standard #: 22VAC40-111-530-A
Description: Based on observations, it was determined that the nap cots were supplied as required. Evidence: The nap cots were not covered with a bottom cover when used by the children.

Plan of Correction: We will begin using a sheet with their blanket.

Standard #: 22VAC40-111-540-A
Description: Based on observations, it was determined that one infant was not securely placed in her bouncy seat. Evidence: The Inspector observed one infant seated in a bouncy seat without the safety strap buckled.

Plan of Correction: The buckle was fastened during the inspection.
