The licensing inspector conducted an unannounced monitoring inspection from 9:30am - 11:45am. There were five children in care (one residential) for a total of seven points with one caregiver. The children were observed playing with the available toys, watching a cartoon, working puzzles and playing outside. One caregiver's record, three household member's records and five children's records were reviewed. Violations from the previous inspection were also reviewed and one violation was repeated. Standard #: 22VAC40-111-60-B Description: Based on record reviews, the family day home failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed five children's records. Two of five records reviewed (child record #1 and #2) had two emergency contacts, but the address for one contact was not completed. Plan of Correction: The provider will ask the parent again for this information. The provider will review the children's records to make sure they contain all required information.
Standard #: 22VAC40-111-240-A Description: Based on observation, the family day home failed to maintain the outside play equipment in a safe condition. Evidence: 1. The inspector observed the outside play equipment. There are two rungs on the ladder to the fort that are loose on the left side. Plan of Correction: The provider will have the ladder repaired. The children will not use this equipment until the ladder is repaired.
