Technical Assistance:
Documentation of children's injuries - both major and minor - was discussed with the provider. Written documentation of all injuries must be maintained in the children's records. Any injury/incident information sent via text message to a parent must have paper documentation in the child's record.

An complaint investigation regarding a complaint received by the department on 6/22/17 was concluded today. The complainant alleged a lack of supervision resulting in numerous injuries to children in care. Observation, caregiver interviews and a review of injury records did not provide a preponderance of evidence to determine the allegation of inadequate supervision to be valid. A violation was cited regarding the required documentation of children's injuries - see violation report. An exit meeting was conducted with the provider prior to closure of the investigation. The Department reserves the right to re-open the investigation should additional information become available. Please contact the inspector if there are any questions.

Description: Based upon review of two injury records, the provider has not ensured that all required information regarding children's injuries is recorded in the child's record. Evidence: 1. An injury record dated 5/11/17 provided for child 1 does not have recorded the time the parent was notified of the child's injury or any future action to prevent recurrence of the injury. 2. An injury record dated 6/9/17 for child 2 does not have recorded the date, time or how the parent was notified of the child's injury, any future action to prevent recurrence of the injury and in not signed by two caregivers or a caregiver and the parent.

Plan of Correction: The provider responded with the following: The injury reporting form will be updated to ensure that all required information is documented n the child's record.
