Technical Assistance:
The inspector will provide the provider with VDSS model forms to aid in the keeping of child records and information. A carpet showing signs of wear is scheduled to be replaced this week.

Comments:
An announced initial inspection was conducted today from 9:30am-11:30am. There were 2 children(4 points) directly supervised by 2staff. The home, 1 staff records, 4 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely. Diapering procedures were also observed. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized with areas of the home labeled in print rich materials. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-111-920
Description: Based on observation, the menu for the current one week period was not posted. Evidence: The family day home provides food for children, but the menu was not posted.

Plan of Correction: The provider will create and post the menu.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 4 of 4 children's records checked were incomplete. Evidence: 1. The records for children 1-4 did not contain contact information for two adults to be contacted in the event of emergency, an information for parents form, proof of identity, and liability insurance disclosure. 2. Child #1's record did not contain the parents' work location and contact number. 3. Child # 2 and Child#3's record did not contain the health insurance policy number.

Plan of Correction: The provider will use the VDSS model forms to obtain this information from parents.

Standard #: VENDHOM-000-016-C
Description: Based on observation, stationary outdoor furniture is currently installed over concrete. Evidence: An outdoor slide is situated to where the trajectory of children would include a corner of the cement patio.

Plan of Correction: The provider will more the equipment to an appropriate location.
