An unannounced monitoring inspection was conducted today from 10:45am-12:00pm. There were 3 children (10 points) directly supervised by the provider and one assistant. The provider is MAT certified. Children were observed playing freely and preparing for lunch (spaghettis, carrots, peas, meatballs, and fruit). The home, staff records, children records, household member records, evacuation drills, and policies were inspected. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Area of non-compliance identified in the Violation Notice. Standard #: 22VAC40-111-100-A Description: Based on review of records, 1 of 4 children's files was missing documentation of a physical examination within 30 days of the child's first day of attendance. Evidence: Child #1 did not have documentation of a completed physical examination before the first day of attendance (May 15, 2017). Plan of Correction: I will have the parent compete this information. They already had the doctor's appointment.
