Technical Assistance:
Reviewed all recent changes to the background check procedures.

Comments:
An unannounced monitoring inspection was conducted today. The provider and one assistant caregiver were caring for 11 children, totaling 19 points. The children were observed playing with toys, singing songs, dancing, and coloring pictures. The children were to be served ground chicken, rice, pasta, tortilla chips, zucchini, corn, and milk for lunch later today. This inspection was completed between approximately 9:30 am and 11:15 am. Violations:
Standard #: 22VAC40-111-830-A
Description: Based on a review of emergency documents, it was determined that the provider has not conducted a fire evacuation drill each month. Evidence: The provider did not conduct a fire evacuation drill during the months of May, June, and July, 2017.

Plan of Correction: I will make sure we do a fire drill every month.

Standard #: 22VAC40-111-50-A
Description: Based on a review of documents, it was determined that the provider has not accurately completed the written daily attendance. Evidence: Upon arrival, there were 11 children present and in care however, the attendance indicated a total of only 9 children in care.

Plan of Correction: The attendance was corrected during the inspection.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that three records were not complete. Evidence: 1. Child record #1 does not include: the parent's medical insurance policy number; documented date of enrollment; a signed "Information for Parents" document; documentation that the provider has viewed the child's proof of ID; a physical exam report; a signed "Provisions of the Emergency Plan" document; a signed "Liability Insurance" document; a signed "Medication Policy" document. 2. Child record #2 does not include: the parent's medical insurance policy number. 3. Child record #3 does not include: the parent's medical insurance policy number; the parent's signature on the registration form; a signed "Provisions of the Emergency Plan" document.

Plan of Correction: All records will be updated and complete.

Standard #: 22VAC40-111-170-A
Description: Based on observations, it was determined that one adult has not obtained a TB Health Screening. Evidence: Upon arrival, the Inspector observed one adult visitor to the home interacting with the children. It was reported to the Inspector that this adult visitor has been staying in the home for approximately 10 days and interacts with the children periodically. This adult has not obtained a TB Health Screening.

Plan of Correction: She is leaving this weekend. In the future I will make sure any adult house guest who interact with the children get a TB screening first.

Standard #: 22VAC40-111-510-A
Description: Based on interviews, it was determined that two children are not provided with a nap space as required. Evidence: It was reported to the Inspector that two children sleep on a soft area made of blankets, but these children are not given a rest mat or rest cot.

Plan of Correction: I have thick rest mats in the basement. I will bring them upstairs and use them.
