Comments:
Today an unannounced monitoring inspection was conducted with Provider. This visit took place in the morning there were 12 children with 3 staff for a total of 28 points within the supervision guidelines. The children were observed playing with blocks, having outdoor play, lunch and in transition for nap. Positive interactions were observed during this inspection. lunch consisted of chicken, spaghetti, broccoli, fruit and milk. A complete inspection of the physical plant, provider records, menu, fire drill log, emergency procedures were observed. Violations:
Standard #: 22VAC40-111-920
Description: Based on menu posted, the family did not have a menu posted for current one-week period. Evidence: The menu posted for today did not have a date and was only for today.

Plan of Correction: Provider stated that she will start doing a weekly menu and will post it tomorrow.

Standard #: 22VAC40-111-50-A
Description: Based on review of the children in attendance, the family day home did not have written record of children in care today. Evidence: No documentation of written attendance for the children in care today.

Plan of Correction: Provider stated that she does the attendance daily however she forgot. Provider corrected during this inspection.

Standard #: 22VAC40-111-240-A
Description: Based on observation of the physical plant, the following areas were found not being maintained in a clean, safe and operable condition. Evidence: In the nap room, chipped paint and holes were found along the wall.

Plan of Correction: Provider stated that she will have it fixed this weekend.
