Today an announced initial visit was conducted with the Provider. This visit took place in the morning between 9:00am-11:15 there were 3 children with the Provider for a total of 8 points within the supervision guidelines. The children were observed having morning free play, dramatic play, lunch and in transition for nap time. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-760-A-1 Description: Based on observation of the first aid kit, the following items were missing. Evidence: the triangular bandages were not found in first aid kit. Plan of Correction: Provider will purchase or add to first aid kit.
Standard #: 22VAC40-111-890 Description: Based on interview with Provider, food is brought from home shall be clearly labeled to identify owner. Evidence: 3 lunch containers were found however they were not labeled with child's name. Plan of Correction: Provider stated that she will inform parents that the lunch containers must be labeled.
Standard #: 22VAC40-111-110-A Description: Based on review of the children records, some records did not include physical exam on the current VA health FORM. Evidence: 3 of the children records did not include documentation of a physical and immunization record on VA health form. Plan of Correction: Provider will ask the parents to fill out the form and get back to her as soon as possible.
Standard #: 22VAC40-111-50-A Description: Based on review of the attendance, the provider did not have written documentation of children in attendance each day. Evidence: No documentation of a daily attendance record. Plan of Correction: Provider stated that she will start to complete it each day.
Standard #: 22VAC40-111-60-B Description: Based on review of the children records, the following information was missing, two or more emergency contacts, parents employer address and phone number. Evidence: Child #1's record did not include documentation of the parents work address, two emgency contact persons address. Child #2's record did not include father's work address and phone number, and 1 emergency contact persons address and phone number. Child #3's record did not include parents work address, and two emergency contact persons address. Plan of Correction: Provider will ask the parents to fill out the state sample form and get back to her as soon as possible.
Standard #: 22VAC40-191-40-C-1-a Description: Based on review of the providers household records, the central registry checks were not completed upon completing application for licensure. Evidence: Household members #1 & #2 central registry checks were not completed. Plan of Correction: Provider stated that she submitted them last week and is awaiting for them to be mailed back. Provider stated that she did not know she needed those.
Standard #: 63.2-1809.1-A Description: Based on review of the children records, the family day home did not have documentation that written notice of liability insurance was provided to the parents. Evidence: Child #1-3's record did not include documentation of written notice of liability coverage was provided to parents. Plan of Correction: Provider will ask the parents to fill out the form and get back to her as soon as possible.
