Technical Assistance:
Suggest placing a child proof latch on the closet near the diaper table. This will ensure the products stored in the closet are inaccessible if older children are in care. The new sworn statement or affirmation was reviewed. New CPS check requirements discussed. Anyone who has lived in another state in the past five years is required to request a check of the child abuse and neglect registry for each state they have lived in the past five years.

Comments:
An unannounced monitoring inspection was conducted on 08/08/2017 from 11:05am - 1:00pm. There was one child in care with the provider. The age of the child counted for three points. The provider was observed engaged in learning activities with this child. Lunch and nap was additionally observed. The downstairs area of the home was reviewed along with the backyard play area. Records reviewed for three children, the provider and one household member. The violations were reviewed with the provider at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-111-800-B
Description: Based on record review, the provider failed to review the emergency plan at least annually and update the plan as needed. The provider shall document in writing each review and update to the emergency plan. Evidence: The written emergency plan for the family day home was last reviewed and updated on June 1, 2016.

Plan of Correction: The provided stated that the emergency plan will be reviewed and updated as needed no later than Monday, August 14th.

Standard #: 22VAC40-111-70-A
Description: Based on record review, the provider failed to ensure that before a child's first day of attendance, parents shall be provided in writing the relocation site for the family day home in the event of an emergency. Evidence: A review of three children's records found all lacking written information informing parents of the relocation site for the family day home in the event of an emergency.

Plan of Correction: The provider stated that the relocation site will be added to each child's written agreement this evening.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the provider failed to ensure that the diapering surface shall be nonabsorbent and washable. Evidence: 1. There were two tears in the diaper changing pad. a. Once the plastic is torn, the surface of the pad is no longer nonabsorbent as bacteria can become trapped in the foam padding underneath.

Plan of Correction: The provider stated that a new diaper pad will be purchased no later than Monday, August 14th.
