The following inspection took place between the hours of 10:30 am through 12:00. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this unannounced monitoring inspection there were a total of 3 children (4 points) with provider. A total of 3 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: toy play, coloring, movie time, bathroom breaks, and hand washing. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-180-A Description: Based on record review, the provider has not maintained a report of Tuberculosis screening for each household member every two years from the date of the last screening. Evidence: Household #1 TB screening is dated 2/23/15, not within the last two years of the date. Plan of Correction: She has an appointment next week for the TB screening.
Standard #: 22VAC40-111-320-B Description: Based on observation, child resistant protective covers were not on every unused electrical outlet. Evidence: Two outlets were found uncovered in the sleeping room and one outlet was found uncovered in the child care area. Plan of Correction: The outlets were immediately covered.
